Which documents do I need to apply for the social supplement?
Famiris automatically grants the social supplement if you are entitled to it. If you are not yet receiving a social supplement but believe you are entitled to it, you can also apply for the supplement yourself.
Contact your case manager or download the ‘social supplement’ application form directly from our website. Submit the completed form:
- By email: info@famiris.brussels
- By post: Famiris, Trierstraat 70, bus 1, 1000 Brussels
Attach your income proofs to the application:
- For employees and international civil servants: a copy of your most recent payslip or a certificate from your employer stating your monthly taxable income.
- For self-employed persons: a certificate from your accountant stating your net monthly income or a certificate from your social insurance fund stating the amount of income on which your current contributions are based.
- For people with a replacement income: a certificate from the paying institution stating the monthly amounts paid.
- For people without income: A sworn statement that you have no income.
- For people receiving a guaranteed minimum income from the CPAS or equivalent: you do not need to send any proof. Just return the completed form.
